This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free. Services Include: Exams digital x-rays cleanings deep cleanings extractions fillings crowns front tooth root canals dentures/partials sealants mouth guards All insurances are accepted, patients need to look at individual plans for coverage details (not all services will be covered by all insurances, copayments may be necessary). Uninsured patients may apply for sliding fee scale based on income. Payment is expected at time of service. Work Hour   Monday-Friday 8:30 AM- 4:30 PM
